I had a heated discussion with a previous boss about a project we were working on and a specific o-ring the design required. He decided to take my prototype directly from the machine shop, assemble it and send it to the customer without me ever seeing it. When the customer returned the leaking prototype, my boss bitched. I showed him that the o-ring he used was wrong and said "this is why the Challenger blew up."
