I replaced a cylinder head on my 99GT due to this issue.

Partially self induced, but a fun job regardless.

Lousy 4 threaded aluminum heads.

I personally never had a single issue with changing out spark plugs on both my 2007 Blown 3V or my 2005 Blown 3V. I used Autolite HT0's which were still 2-piece plugs and not a single one after about 10 changes between both cars were done. I think it is really user error most times.

I would soak my plugs in penetrating oil for a good 20 minutes to help loosen up any dirt to reduce tension while attempting removal of the plugs. Then slow turns with minimal pressure to get it loose. Plugs are not perfect, but, I believe it is one of those things that needs to be done very carefully.

When I worked for the city we had several of the Triton motor break a plug. We did everything including penetrating oil and nothing worked on the *bad ones*. Ford service recommends something silly like soak for 15 mins then 1/8 turn. Repeat 4x times. That failed too. Some trucks were fine no issues. But the few that were problems were enough to cause hatred of the Triton motor.

Consider yourself lucky I guess. It's not a user thing - its a piss poor OE design.

Piss poor OE design? Sure.

I'd say after 10+ times of doing them on my own 2 cars and helping a large number of friends with their 3V mustangs it wasn't all luck (respectfully, of course)

Penetrating oil + warm engine = "lucky" installs for me.
