I've highlighted "what's missing". This is key in making improvement in any aspect of "exercise". How bad do you want it? And are you willing to do what it takes....

Like I requested earlier, post a video of you benching. And post what you do on bench day (I don't have a "chest" day, I have bench days).

But if you've been training you are right you are missing something. Could be some or all of the below....(like blake-b also stated)

Overtraining (although I don't really believe in this IF you have deload weeks)
Undertraining
Poor nutrition/recovery.....

But if your bench really lacks it's most likely a lack of tricep and upper back strength.
