Loose gas caps = gas fumes leaking into the air = pollution = fail. Either this guy's fuel tank system has a real problem and filling the tank just confuses the computer or hides the problem temporarily, or he is too stupid to put his gas cap on correctly. Either way, he should be punched in the face and permanently failed.
