I'm about 95% sure it's mange.
There's a mite called Demodex that burrows into the hair follicles of dogs. Take her to the vet, have them do a skin scraping (it'll look even worse after they do it since they have to scrape until they draw a little blood), they'll take a peek under the microscope, and if they see a little bug that looks like a cigar with legs, that's Demodex. Causes Demodectic Mange.
They'll give you something for it, maybe Ivermectin.
