Yeah you do have to be somewhat careful with tendons. If they get messed up it can be a long road. My buddy hurt his Achilles tendon at the gym, let it heal up for 6 weeks then snapped it climbing on to a boat and was out for a year.
I'm not sure about the bike. The doc probably won't know either unless he's a sports doc. If he can bike with the boot on then maybe, but if it hurts he'll have to stop.
The upper body weight lifting is a no brainer though. If he's putting on weight then he needs to reduce the calories while he's not working out to head it off at the pass.
