in the Civil War series, Nitro (who was hyped up on MGH) was attacked by Wolverine and a group of shield agents and consequently blew himself up (his power btw) and completely incinerated Wolvie down to his adamantium skeleton. at which point his body did die and his soul went on to battle a mysterious character (which happens every time his body "dies") and as he kills the shadowed nemesis, it turns into Jean Grey, or one of his other numerous loves over the centuries. after which point, Logan comments that he has to fight the guy every time his body dies, and as long as he kills him (and parts of his own world in the process) he can once again return to the land of the living. but if he were to ever lose that fight, then he would actually die.
heres the back plot for that:

during WWI Wolvie fought and killed a man he thought to be a german soldier named Lazaer, later revealed to be Azrael the angel of death. he came after Wolvie because "he defies the natural order of things" Apparently the prize you win for killing Azrael is a chance to free your soul from the afterlife. Because he already killed Azrael on this plane of existance, he battles wolverine in purgatory whenever he is injured to the point that he "should have died"
