It could have been prevented, it's just that once Mission Control told Columbia that they're go for de-orbit burn, nothing could be done.

So what would have happened if STS-400 had taken place, but Atlantis re-entered fine?

Atlantis wouldn't re-enter fine. Mission Control would purposely make her break apart.

I never new about them flipping it to take shots.

That's what I call HIGH res photos. Pretty amazing.
